Pursuant to Washington State’s Safe Start requirements, we are required to request the contact information of dine-in guests to assist with contact tracing efforts. Your participation is completely voluntary, but we support efforts to help reduce the spread of COVID-19 in our community.
We have provided this online form to provide your contact information. We will never share, sell, or use this information for any purpose other than to assist in contact tracing efforts with local health officials. Per state guidelines, after 30 days your information will be permanently deleted.
